Terms and Conditions
DDS Perks is a dental membership plan and not to be considered dental insurance. The dental membership plan is for individuals without dental insurance and cannot be used in conjunction with a dental insurance plan.
Eligibility begins upon payment of the $99 enrollment fee and remains active for 12 months from enrollment date. No treatment exclusions. No waiting periods.
Membership will automatically renew on the anniversary date unless notice of cancellation is given 30 days prior to renewal date. Dues are subject to change annually.
Only valid at Distinctive Dental Services in Lake Jackson, TX. Care from other providers or specialists is not included.
Members’ accounts must have zero balance before enrolling.
Plan is not retroactive.
It is the member’s responsibility to utilize services included in this agreement within their plan’s year. Any unused benefits will not be carried over or refunded.
This plan is non-refundable and non-transferable.
The member agrees to pay for all balances in full at the time of treatment. If treatment is not paid for at the time of service, any discounts can and will be voided.
If Care Credit or Lending Point is the payment method of choice, the 30% discount on preventative appointments will be reduced to 20% ($279/visit) and the 15% discount on dental treatment will be reduced to 5%.
15% off dental treatment does not include Invisalign or 6 Month Smiles.
15% off can not be combined with any other offers or promotions.
Fees for dental services are subject to change at any time. Treatment plan estimates will be honored for 90 days.
Members must be current and active in the DDS hygiene program in order to receive plan benefits. This includes accepting, scheduling and receiving any recommended periodontal (gum) therapy.
Members must agree to schedule their next preventative appointment.
Members must give at least one week’s notice of appointment cancellations or a $75 cancellation fee will be applied.
Membership can and will be terminated if a member is found to be in violation of any of these terms and conditions. There will be no refund of enrollment fee.
